All three sinks in the room were being used for mulitpurpose sinks with no dedicated sink for hand washing. This was corrected when the director had the caregivers clear a sink to be used only for hand washing.
The fence surrounding the play area was observed to have approximately 6in opening at the bottom of the fence.
