Wilson Head Start Campus

An ISD classroom observed that is llicensed for headstart use. Room has changed to ISD use since school year started on August 18th. Notice of space reduction was not sent to licensing. Children in room are not enrolled in headstart program.

Children in headstart licensed area that is now ISD room are not enrolled in headstart program. Children Records were not available for review.

Based on the preponderance of evidence obtained it is determined that the medication was not administer according to labeled directions. Medication indicated to be given at AM and PM times, at operation the medication was given twice in the AM within a time lapse of an hour and a half.

Based on the preponderance of evidence obtained it is determined that the operation was not keeping record according to policy which requires to write down the medication dosage administered at time when medication was given.

Six staff files were reviewed and are missing items including personnel information and training records.

Operation's playground has a play-structure with a highest designated play surface of 8ft (operation will service children ages 3-4yo). Structure does not have the required 9in of surfacing.

Red ants were observed inside classroom #3 and operation's playground.

First Aid kit is missing the following items: Guide, Antiseptic solution / wipes, Thermometer and Tweezers.

Current daily schedule does not meet the following requirements: Naptime, 60 minutes of daily outdoor play and 90 minutes of daily physical activity for prek-age children.
