During review the operational policies were found to be missing the promotion of indoor and outdoor physical activities.
During the inspection personnel records were observed and the CPR/First Aid trainings completed by 4 out of the 6 records seen did not meet the requirements for use of a training manikin and hands-on skills-based training.
During the inspection it was found that the operation had not had a fire inspection completed since Feb. of 2021.
During the inspection a caregiver who was present during the walk through was found not to have had a background check submitted for the operation.
During the inspection the playground was observed. The operation had started to make changes to the outdoor size without informing CCR. This was corrected at inspection when the operation discussed the changes being made.
During the inspection I observed two play structures that required 6-inches of loose fill. However, the loose fill had packed down and could not be measured at 6-inches.
During the inspection I observed two caregivers not wash the infants hands after a diaper change.
