At the time of inspection the operation's first aid kit did not include a first aid guide.
At the time of inspection the operation's evacuation/relocation diagram did not indicate a shelter in place location. Caregivers could not identify a location in the building for sheltering in place.
At the time of inspection it was observed that the operation had reduced space due to renovation of the blue room to build classrooms. According to staff the program has not had access to the blue room since the beginning of the school year. In addition, the program is no longer using the same restrooms as new restrooms were constructed in the cafeteria.
At the time of inspection it was observed that children were not required to wash their hands after coming in from outdoor activities. This was corrected by caregivers having all children wash their hands before beginning indoor activities.
At the time of inspection the operation did not have 4 shelter in place/lockdown drills documented for 2024. Caregiver informed that only 3 drills were conducted due to not having access to a designated space.
At the time of inspection the operation did not have a system for storing children's belongings. Backpacks and jackets were observed at multiple cafeteria tables in no specific order and hand no identifying labels.
At the time of inspection a child was observed going to the bathroom without notifying staff. Staff were not aware that the child had left the group.
At the time of inspection there was no documentation available for emergency practice drills.
At the time of inspection one child did not have consent for medical treatment as part of their admission record.
At the time of inspection, the operation's liability insurance had expired on 12/31/2022.
At the time of inspection, the operation did not have required phone numbers posted. During the inspection, peration staff was able to complete the list of phone numbers to add to the program's board with other required postings.
At the time of inspection one caregiver did not have a cleared background check submitted for their designated location (Liberty Elementary).
