No fire drills have been documented for the months of September and October.

There is no monthly checklist for September and October.

The program has not had a designated director since 01/04/2024.

A 9 year old girl was allowed to go to the restroom by herself. She left the waiting area and closed the door. The restroom is next door in the gym. No staff went with her.

There were 14 children in care. Three children were sitting on table tops. All the other children were sitting in chairs. There were not enough chairs.

The provider was contacted on 01/27/23 and 02/03/23. Documentation was requested showing the applicant meets director qualifications. The documentation has not been submitted.

Two children enrolled with food allergies do not have allergy plans on file.

Based on the information gathered staff did not show good judgement when the staff person went against policy which was not to play with children while the children played soccer.

The program has not had a site director since Feb. 17, 2022.
