YWCA Glen Cove After School

During this inspection, I observed the staff caring for children in a new space that is unlicensed. This was corrected once the area was inspected, measured and photos were taken.

Site Director is missing 15 annual required training hours for 10/18/2023 to 10/18/2024.

Two staff members were missing their file at the operation.

One child was missing the information of Name, address, and telephone number of another responsible individual (friend or relative) who should be contacted in an emergency when the parent cannot be reached.

Two children did not have the information of names and telephone numbers of persons other than a parent to whom the child may be released on file.

The first aid kit was incomplete; a pair of tweezers were missing in the kit. This was corrected once the site director provided a pair that she had extra.

One child did not have information as who the child can be released to in case the parent or guardian is not able to pick up the child.

One employee did not have on file documentation of receiving orientation.

One child did not have emergency contacts listed on the admission form.

Two caregivers did not have their annual training of focus on prevention, recognition, and reporting of child maltreatment. One hour is required yearly.

Documentation of a site director has not been submitted to the Licensing office. Currently the operation does not have a site director.
