The director was missing annual training on the following topics: (1) Recognizing and preventing shaken baby syndrome and abusive head trauma; and (2) Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS);

The director did not have current training in Transportation Safety. The transportation safety training certificate available for review had a date of completion of 5/1/2025.

Two vents in the toddler room were observed with dust and debris.

Three out of six children present were not signed in. Corrected at Inspection: The times and initials were placed for those who had not been sign in.

A medication liquid bottle and hydrogen peroxide bottle were observed on a shelf in the cafeteria area which are easily accessible to the children.

The outdoor area was observed with ants, nail, screw, feather, 3 torn pieces of zip ties, and Q-tip cotton swab.

An 11 month old infant was observed asleep on a bouncer. The infant was observed with a bottle and covered with a blanket.

There were two left over bowls with cereal on the table which were accessible to the children in the classroom. Corrected at Inspection: The bowls were removed.

The director did not have a transportation training certificate.

The sign in and out log was not current. One child was not signed in. Corrected at Inspection: The sign in and out log was updated during the inspection.

There was no documentation in the children's files to demonstrate the notification to the parents of no liability insurance in the operation.

A caregiver hired on 2/20/23 did not have the 24 hours of Pre-Service Training.
