At inspection, four caregiver files did not have the required 2912, pre-employment affidavit form and one caregiver file was missing the 2985 notarized affidavit form.

One child did not have a food allergy emergency plan signed by a healthcare professional.

One child's file was missing a health statement.

The operation has a climbing structure with a slide that does not have proper use zones. The slide has a climbing wall in it's use zone. The use zone in front of the slide does not contain a full six foot zone of surfacing material.

A 10 month old infant was sleeping on a cot at nap time. The child is not able to walk yet. The child was moved to a crib during the inspection.

6 staff were missing the following required content trainings: SIDS, Admin Medication (except 1), Food Allergy, Hazardous Material (except 1).

2 out of 6 children were missing some of their shots.

2 out of 6 staff were missing their First Aid and CPR certificates.

One file of four reviewed needs to have the high school diploma translated. Two files were missing copies of the high school diplomas. One file was missing a copy of the First Aid/CPR certification which has already been taken.
