Children came in from swimming, and went to right to playing. The children were brought to the carpet after about 10 minutes, and were broken into groups to change into dry clothes. No mention of washing hands was made. When I asked one child if he was going to wash his hands, he went and washed them but there were no paper towels in the men's room, so I went into the lady's room, there were no paper towels or hand dryers there either.
The children were on a field trip but were not wearing an identifying item with the operation's name and number.
One of the vans used to transport children did not have a front license plate. This would not be considered maintaining the van safely.
The caregivers did not know how many children they had in care. When asked, they told me 15. When we counted together, it was found that there were 13.
There were 34 students and 4 teachers with the youngest child being 4 years old.

One caregiver did not have the required 15 hour of annual training, only 12 hours were accounted for.
The program director had ten hours of the annual training, not the 20 required.
When children were transitioning from outside they did not and were not instructed to wash their hands.
There is a hole in the wall on the ramp in the room used by the children, measuring more than 12 inches in length and 10 inches in width. The hole exposes ventilation and the inner components of the wall.
A caregiver was alone with children, despite having conditions on his background check stating that they cannot be alone with them or transport children due to their provisional status. HHSC will recommend an administrative penalty as a result of this citation, per HRC Section 42.078.
There was a child in care at the time of the inspection who did not have any records. The child also had a known allergy.
There were two children with known and listed allergies that did not have a completed food allergy plan in their records.

The maximum height of the highest designated play surface on a climbing structure used by 4 year old children is over 5 feet high.

Two caregivers were present who did not have eligible background checks. They both left during the inspection.
The use zones for the climbing structure and the swings do not have an acceptable depth of uncompressed loose fill surfacing. There is a lot of grass growing up through the surfacing that has compacted a lot of it.
Reviewed 45 children s applications with the director's and parent's signature. The children from the day care home applications were not on file with Club Z.
There were 3 individuals present supervising children without a background check and requested to leave the premises. 2 other caregivers arrived from another Club Z operation, the operation was in ratio.
