Three infants in the infant classroom were observed to be watching television. This was corrected at inspection when the television was turned off.
A caregiver was observed leaving the classroom, leaving infants unattended.
There was no loose-fill in the use zone for a piece of rocking play equipment. This was corrected when the piece of play equipment was removed from the playground.
The operation was missing policies for hearing and vision screening requirements, procedures for parents to review minimum standards and the most recent inspection form, procedures for parents to contact the local licensing office or the DFPS abuse and neglect hotline, the criteria used to determine extreme weather, and a plan for physical activity to be done indoors if extreme weather prohibits outdoor play.
The operation did not have a signed form to certify that the children's products recall list had been reviewed. This was corrected at inspection when the director signed and dated the form to certify that she had reviewed the recall list and no recalled products were used in the center.
Out of five children's records reviewed, two were missing a statement from the parent that they did not have food allergies, two had outdated health statements, and one was missing an immunization.
A table on the playground had one of the covers pushed to the side allowing water to enter inside the table that was accessible to children in care. This was corrected at inspection when the director had the maintenance man empty the water and remove the table from the playground area.
Classroom 1 was out of ratio by 3 children as their were 7 young children total with 1 caregiver and 1 volunteer. This was corrected at inspection when we had another caregiver come into the room.
There was no loose fill at the end of the slide. This was corrected at inspection when the Director had the maintenance man remove the slide from the playground area.
During the inspection a 7 month old was observed sleeping in a bouncer inside the crib with a blanket. This was corrected by the bouncer, the blanket, and the infant being removed from the crib and infant placed backed into the crib with tight fitting sheets only.
