The transportation van was not clean and filled with trash, paper, debris, and other clutter on the floor of the van.
The base board in the Pre-K children's restroom is damaged and needs to be replaced.
An admission form for a school age child did not include the school's name and number.
Window cleaner was observed in the Pre-K children's restroom and was accessible to children. This was corrected at inspection when the window cleaner was made inaccessible to children by removing the cleaning product from the cabinet.
Based on documentation obtained during the application inspection on 12/27/23, the operation has not received a required fire inspection conducted by the local fire marshal.
The outside small playground has a wooden board missing from the fence creating an opening in the fence big enough for a child to go through.
