It was found that the number of employees who ceased working at the operation in 2024 was not reported through the provider portal by 1/15/25.

Four out of eight children were missing a date of admission.

One out of eight child records reviewed is missing the medication authorization.

During the walk through it was found that the outside AC unit is accessible to children and does not have a safe guard.

One out of four personnel records reviewed is missing the education qualification. Note: Caregiver lost the high school diploma during a flood. Information was given on how to request a copy.

During the time of inspection a 19 month old was watching TV. This was corrected when the director turned the TV off in the classroom.

Written activity plan is lacking 90 minutes of moderate to vigorous play.

Infant's daily sheet is not being shared with parent's daily. This was corrected when the director printed out infant care sheets for each child.

Carbon monoxide was unplug and not in use during the time on inspection. This was corrected when director plugged in the carbon monoxide and replaced batteries.

In the Pre-K class the TV is not anchored to prevent from tipping over. This was corrected at inspection when the director removed the television from the classroom.

During the time of inspection a caregiver was supervising 4 children ages months to 23 months. There were 34 children in the building. This was corrected when the director removed the child from the infant class and put the child in the 2's class.

Infant's crib has a supplemental mat under the mattress to raise the child when sleeping. This was corrected at inspection when the director removed the supplemental mat.

Carbon monoxide detector was checked in August 2021. This was corrected when the Director put new batteries in the detector and tested it at inspection.

Handwashing sink was being use to clean training cups and bottles. This was corrected at inspection once director was made aware.

Director stated that they have not been conducting a monthly checklist for maintenance.
