One staff CPR/First-aid certification expired July of 2024. The staff was observed present, and/or caring for children. An additonal staff file observed did not have CPR/First-aid certification available for review.

During the annual inspection, the site Director training certificates, as well as staff training certificates were not available for review. The staff observed was able to portray certificates on a computer for Licensing to observe

Two out of five staff files did not have the required 15 annual training hours. One staff file observed contained 3 out of the required 15 hours. The second staff file observed contained 11 out 15 total annual hours.

One out of five staff files observed did not have a notarized affidavit available for review.

Two out of four employee records observed did not include a notarized affidavit. A notarized affidavit was not avaible for review during the annual inspection.

Three out of four employee records did not include a start date, and/or employment date.

One out of four staff records observed did not include a CPR/First-Aid certification available for review. The Director record did not include a current CPR/First-aid certification.
