The previous Director last day employed was 03/07/2025. Licensing staff was not notified of change until inspection date, 03/27/2025. A new Director has been hired.

One individual hired through a substitute service was observed in the Toddlers classroom with 9 children, 2-3 years of age. The individual did not have an active background check, and/or a file to verify for Licensing staff to review. The individual was removed from the facility.

One staff member last day of employment was 03/07/2025. As of 03/27/2025, the employee background check still portrays active in the CLASS system.

The employee list, and/or background check list has not been validated since February 27, 2024.

The most recent annual sanitation inspection was dated 06/14/2022. The documented expiration date is 1/07/2023.

Two staff observed in the First Steps classroom was not aware of the age of the youngest child in care. A total of 6 children were observed with two caregivers. Ages of children were verified when assistant Director arrived at 9:52AM.

The playground structure had nails protruding from the play structure. The play structure posed as a risk to children due to broken boards near the walkway. The staircase observed was broken and leaning towards the ground.

The operational policies did not include information related to the promotion of the physical activities. The operational policies did not include information related to outdoor structured playtime.

After outdoor playtime, the toddler/pre-k age group was observed walking into the building and then proceeded to drink from the water fountain. The children were not observed washing their hands after outside playtime.

Expired medication was observed in the Infant 2 classroom. The medication was observed in a refrigerator that was molded. The refrigerator and medication were removed from the infant 2 classroom.
