I did not observe documentation of current transportation safety training for a staff member who transports children. NOTE: This was corrected during the inspection.
Interviews were conducted and documentation of the incident was evaluated. It was determined that a toddler was left alone on the playground when the two caregivers who were working with the group did not follow the operation's procedure for transitioning children from one area to another. NOTE: Both caregivers were terminated after the incident and remaining staff were trained on new procedures for transitioning children.
