Diaper changing pad was observed to have a rips and tares in the middle and on the side of the diaper changing pad exposing the foam. Corrected at Inspection- the diaper changing pad was replaced during the inspection.

1 out 10 staff records evaluated did not have a current DL in the file. Corrected at Inspection: the director was able to get the current DL from the caregiver.

Feeding instructions for three infants was observed to not be updated for the required every thirty days.

10 out of 10 staff files did not have the preemployment affidavit made available for review.

Based off the information from the DFPS investigation, it was determined the operation did not follow background conditions for a caregiver that is employed at the operation.

Ten children records' evaluated did not have the parents right form in the file.

Children were observed to not wash their hands before eating.

Hand washing was not observed in two different classroom after a diaper change.

Caregivers in two different classrooms were observed to not wash their hands after diaper change.

The operation failed to follow user instructions when cleaning. The cleaning product stated the chemical should not be used on food contact surfaces but was used to clean inside the kitchen.
