Employee's files were lacking of pre-employment affidavit form 2912.

The operational policies were lacking of the parent's rights information. Form 2987 was not available neither.

A review of the Employees Who Left Employment in 2024 data revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

Based on information obtained during the investigation two caregivers employed for over 90 days as of July 30, 2024, did not have current first aid and CPR training.

Based on investigation findings a caregiver inappropriately disciplined a child in care.

Two caregivers did not know the age of the children in their classroom.

The fire inspection was last conducted October 26, 2022.

Diaper changing pad was torn and ripped in the Pre-Toddler classroom.

A dirty diaper was left on the diaper changing pad in the Toddler classroom during inspection. The caregiver was not changing diapers at this time.

Two emplyees were at the operation without clear background checks. A caregiver was seen monitoring children outside with no background check. A caregiver was seen in a classroom while children were seated for circle time.

A caregiver was seen sitting in a chair while children were jumping off of a ledge on the play area. The caregiver stood near the ledge as the children played to see all areas of the playground.

A volunteer employed by the center pushed a child in care to the ground.

Staff in the 1 year old classroom did not know the correct number of children in their care.
