It was determined that a person hired was no longer employed as of 8/16/2025 and the person was not inactivated in the required time frame.
During the investigation it was determined that a caregiver was observed on video picking up children roughly and some under one arm to move them. The operation immediately dismissed the caregiver and reported the incident to CCR.
During inspection an infant room was out of ratio when the second caregiver stepped out of the room to handle business.
At the application inspection the completed Health Inspection for the new operation could not be observed.
