The infants in both infant rooms were sitting at the table. The infants were not strapped in the seat. This was corrected during the inspection when the caregivers strapped the infants in the seats.

A Review of the Employees Who Left Employment in 2024 data from January 22, 2025, revealed that the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

A plug was uncovered at inspection. A plug cover was installed during the inspection.

The operation did not provide adequate lighting during naptime. When we arrived, we had to use a cell phone light to see in the rooms. We spoke with the teachers and director about using lamps and uncovering the windows to let light into the rooms.

The operation did not have current training in transportation.

Cleaning products were sitting on the counter in reach of the children. The products were moved to a location children could not reach during the inspection.

Two toddler classrooms did not participate in morning outdoor activities.

The infant rooms did not have daily sheets available at inspection.

The diaper changing mat in the toddler room was torn, exposing the underlying foam.

Several nap mats were torn, exposing the underlying foam.

Four infants were observed sitting in a restrictive device for an extended period of time.
