Observed an infant sleeping in a crib that did not have a fitted sheet on the mattress. The infant was sleeping on the mattress. This was corrected when the infant was removed from the crib.

During an investigation it was found that the operation had 14 children in care in one room where the youngest was 4 months old and the oldest was 5 years old. There was more then an 18 month gap between the two children.
During an investigation it was found that a classrrom was out of ratio. There were 14 children for 1 teacher and the specified age group was 18-23 months which allows 1 caregiver to have 9 children.
A caregiver in one classroom stated that there were 10 children in their care, but the caregiver actually had 11 kids in care.
Observed an infant in a play yard that had a hole in the mesh lining of the play yard. This was corrected when the infant was moved to another empty play yard.
All caregivers who were in the building during nap time was counted in ratio. There was not an extra person in the building.
