During inspection on 8/4/2026, the inspector observed in the infant room a 9-month-old child sleeping in the crib with blanket inside. Note: this was corrected when caregiver removed blanket during inspection.

During inspection on 8/4/2026, it was determined the background check for a volunteer who is assisting in the classroom was not cleared.

Based on the information obtained, it was determined caregiver did not provide supervision when a child walked out the child-care center unsupervised.

Based on the information obtained, the operation did not provide an Incident/Illness Report to the parent within 48 hours of the incident.

One personnel file evaluated was missing training record, notarized affidavit, and orientation documentation.

I observe a fan in the pre-k and school -age rooms sitting on the floor accessible to children.

The toilet seat in toddler restroom was loose presenting safety hazard.

In kitchen next to the infant room the garbage was accessible without a lid. Note: This was corrected when the director removed the trash and replaced it with a lid.
