Caregiver was unaware of which children they were responsible for, resulting in a child being bused to the wrong school.
Based on interviews obtained during the investigation, it was determined that a caregiver placed a child in a trashcan as a form of discipline one or more times.
Child was given medication that belonged to another child by a caregiver.
There was a vinyl mat in the infant room that was torn, a vinyl climbing pad in the older infant room that is torn, two window screens that are accessible to children that are torn, and three vinyl couches torn with foam exposed. Photos were taken.
A four-year-old child was unsupervised in the classroom for approximately 3 to 4 minutes. The child stated they went inside the building to use the restroom.
The operation had a staff member test positive for COVID and did not notify licensing within the required time frames.
