The allegation is related to a DFPS abuse/neglect investigation. Based on the information received during the investigation conducted by DFPS, a dosage medication wasn’t administered.

Documentation of fire inspection is need for recent inspection.

One child's file infant feeding instructions were last updated on 4/27/23.

Four out of ten staff files did not have notarized affidavits available for review.

3 out of 12 classrooms had children with their heads covered with their blankets during nap time.

Loose fill in the 4 plus playgroung has less than 6 inches in loose fill.

2 out of the 8 children in the infant classroom infant feeding schedule was not updated since June 2022.
