Three out of the ten caregiver's annual training hours that was reviewed had less than 24 training hours.

There was no lock- down drill documenation to review at the time of the inspection.

The playgound has not been documented since 7/10/22. No updated documentation was available for review.

One out of ten children's records did not have immunization record available for review. Child was enrolled on 11/8/18.

The former's director last day was on 7/28/2023 and the operation failed to notify Licensing within 5 days. Licensing was notified on 9/21/2023.

Two infant seating equipment with straps had infants with no straps on.

The 3-6 year old playground had less than 6 inches of loose fill (wood chips).

The two infant classrooms had children sleeping in restrictive device (bouncy). The ages of the children were 17M, 6M, 11M, 6M,and 11M.

A staff was observed not wearing gloves during diaper change.

Three classrooms out of eleven did not know the number of children they were responsible for.

A teacher in the infant classroom did not have an available SIDS/safe sleep training available for review.

The operational policies were missing a few items to include the promotion for indoor/outroo physical activity.
