Training certificates were not available regarding infant training for the caregivers who were working with the children under 24 months. Training certificates were also not available for the Director and driver, related to transportation training.
The picnic table on the playground had a board which was loose. This could cause a pinching hazard. The board was replaced during the inspection. Pictures were taken.
Two children's records were lacking the school informaiton for a school-aged child. They were both corrected during the inspection.

This standard was evaluated regarding a report received and found to be deficient. Evidence supports that the operation's van driver failed to properly check the vehicle which resulted in a child being left unattended in the vehicle. Evidence also supports that the operation driver was allowing children to push the child safety alarm to shut it off.
DFPS investigator received information to support a child in the care of the operation was neglected.
This standard was evaluated regarding a report received and found to be deficient. A caregiver failed to properly check the operation's van after picking the children up from school. A child fell asleep on the van and was left unattended.
