Last fire drill and CO detector and Fire extinguisher checks were documented on 10/04/2023. Per provider fire drill and CO / Fire extinguisher checks were conducted 11/21/2023 but were not documented. Note: This was corrected at inspection when provider documented the drill and checks under the emergency practices form.

One caregiver was supervising a group of five children ages 10mo, 10mo, 11mo, 13mo and 13mo. Classroom was over ratio by one child. Note: An additional caregiver was assigned to infant room during inspection.

There is a play structure with a slide surrounded by 2-3 inches of pea gravel. Structure's side which holds the staircase is within 3ft from the fence. There is a spring bouncers structure surrounded of 4 inches of pea gravel. Note: This was corrected at inspection when provider removed both structures form operation.

Last Fire Inspection Report available for review was conducted on 11/2020. Per provider; a fire inspection was conducted on 12/2021 but report was not available for review at the time of inspection.

One of the hinges of the door diving prek and school-age playground is out of place making door unstable and difficult to open.
